Must be a Registered Dietitian with a Bachelor’s degree in Nutrition or Public Health Nutrition; 3 yrs exp in clinical or public health. Previous home care exp & Master’s degree preferred.

Seeking a full time RN to provide hospice & palliative care to seriously and terminally ill patients. Responsibilities include: providing skilled nursing assessment, planning, and care in order to maximize the comfort & health of patients and families; providing direct nursing service & nursing case mgmt; developing/implementing a comprehensive plan of care and revising/updating as needed; discharge planning.
