The New Jersey Department of Health has partnered with Docket® to allow easy access to personal and family immunization records using the free Docket app. After a simple sign-up, you’ll have your immunization records right on your phone. Click New Jersey’s COVID-19 Information Hub for more information and to download the app. NOTE: New Jersey residents must have a valid email or phone number on file with New Jersey Immunization Information System in order to access personal or family COVID-19 immunization records using this app. Please consult with your healthcare provider or health department if you have trouble locating your records using this service.
