Intravenous Therapy
VNA of Northern NJ (VNA) home care team includes specially trained professionals who can provide infusion services for patients who have been prescribed IV therapy as part of their home care plan. Wound, Ostomy, and Continence Management Certified wound care nurses are available to provide patient education, consult with other professionals and assist patients and family members who serve as primary caregivers with wound and stoma care, ostomy and continence management. Nutritional Counseling The VNA home care team includes registered dietitians. Upon the recommendation of a visiting nurse or a client's doctor, in-home consultations are available to assist in variety of ways: |

Medication Management
Medication management is a major concern for older Americans. In fact, more than 87% of people over age 65 take multiple prescription medications and people over age 75 take an average of eight prescription drugs on a regular basis. VNA provides one-on-one visiting nurse consultation programs to help prevent dangerous health implications that can result from inadvertent misuse of prescription medications. Telehealth - Connecting Patients at Home with Their Health Care Team
VNA's Telehealth Program monitors a patient's vital signs and symptoms daily from the comfort of their home and transmits the information directly to their health care team.
Through Telehealth, VNA is able to provide personalized, individual care while allowing patients to live more independently with confidence. With Telehealth, VNA can provide better patient care, assist patients in avoiding hospitalizations and pinpoint the best time for a home visit.
Eligible patients are loaned, at no cost, a Home Monitoring Device that sends daily health information automatically and securely to VNA. Once received, the information is reviewed and the patient's home care team at VNA will adjust the plan of care, as needed.
VNA has partnered with Cardicom®, reputable developer, manufacturer and solutions provider of clinical telehealth services, to provide patients with this service.
