Joint Replacement Home Care Program with Direct-to-Home Discharge
Ideally suited for patients who are in otherwise good heath, VNA’s intensive, in-home physical therapy visits allow patients to completely bypass or significantly shorten inpatient rehabilitation confinement with the permission of their physicians. A VNA nurse who will also serve as your case manager will assemble a team including a physical therapist, occupational therapist and home health aide visits as needed.

Rehabilitation: How it Works
- Care begins within 24 hours of returning home
- Special equipment required for your recovery, such as a hospital bed, portable commode, shower bench and other assistive devices, will be ordered and put in place prior to your arrival
- One-on-one sessions with skilled physical and occupational therapists tailored to each patient’s needs and adjusted to keep pace with the patient’s progress
- Certified home health aides trained in post-orthopedic surgery care will assist with essential personal grooming and light housekeeping tasks
- Therapy sessions will decrease in frequency as the patient improves and regains their ability to leave home independently
